18-24 Learning Disabilities Team
The 18-24 Learning Disabilities Team is a specialist team supporting and safeguarding young people from 18 years to 24 years who:
- have a diagnosis of a learning disability; learning disability and autism; or a learning disability and a physical disability
The team's role is to support young people and their families, to build sustainable long-term solutions to their care and support needs by focusing on enablement and independence. All work is person-centred and strengths-based. This means the young person and their family play an active role in developing a support plan which will enable the young person to achieve their aspirations and be part of their community.
The team's role includes carrying out Care Act Assessments, Mental Capacity Assessments, Community Treatment reviews, Adult Safeguarding, working jointly with the Care Leavers Team, supporting colleagues in SEND Education to develop Education Health Care Plans, working with carers and Court of Protection work.
We work as part of a multi-disciplinary team and work closely with our partners in:
- Disabled children’s services
- SEND education
- Leaving care
- Adults with learning disabilities
- Health partners
The team work with young people who are eligible for social care.
